The observatory is backed by the six university affiliated (CAU) health and social services centers (CSSS):
- le CSSS-IUGS (Sherbrooke)
- le CSSS Bordeaux-Cartierville—Saint-Laurent
- le CSSS de la Vieille-Capitale
- le CSSS Cavendish
- le CSSS Jeanne-Mance
- le CSSS de la Montagne
The CSSS-IUGS assumes the leadership role in the development, coordination and administration of the OQRLS. CSSS Bordeaux-Cartierville—Saint-Laurent shares this position jointly. Observatory funding is granted by the Ministère de la Santé et des services sociaux.
The observatory seeks experiments in which the CSSSs and their partners jointly develop initiatives which have both positive and concrete impacts on local populations. The observatory analyzes and compares the initiatives in order to disseminate useful knowledge and experiences to all local networks throughout Quebec.

- Sherbrooke and affiliated regions (Bas Saint-Laurent, Mauricie-Centre-du-Québec, Montérégie Nord-du-Québec/Nunavik/Terres-Cries-de-la-Baie-James, Saguenay-Lac-St-Jean).
